How to Refer a Patient
We accept direct referrals from outpatient providers, EAPs, sober living homes, hospitals, detox centers, and other programs. To make a referral:
1. Contact admissions
Contact us at 512-559-8534 or admissions@plumcreekrecoveryranch.com. We’ll review availability and determine fit.
2. Submit documentation
With consent, share relevant clinical records and assessments.
3. Coordinate care
We’ll keep you updated and plan together for post-treatment care when appropriate.
If Plum Creek is not the right fit, we’ll assist with alternate referral options whenever possible.

Yes. With client consent, we’ll provide updates and coordinate care throughout treatment and at discharge.

At minimum: recent clinical notes, history of substance use, any co-occurring diagnoses, and current medications. We’ll guide you through anything additional we may need.

Yes. Once a release of information is signed, we can coordinate directly to receive medical or clinical records by secure email or fax.

We can typically assess and admit within 24–48 hours, depending on medical needs and bed availability.

We consider patients on MAT on a case-by-case basis, depending on the medication, dosage, and treatment goals. Please contact our admissions team for details.

The average stay ranges from 30 to 45 days, with flexibility based on clinical need, progress, and insurance authorization.

While we are not a gender-segregated facility, men and women are housed in separate wings, and we incorporate gender-responsive topics into the group and individual therapy process.

We work with referring professionals to ensure a smooth handoff into and out of treatment. Discharge planning begins early and is tailored to each patient’s needs and external supports.
Collaboration with outpatient providers
With patient consent, we maintain open communication with outpatient therapists, psychiatrists, and case managers. Our team shares relevant clinical insights and works toward a shared understanding of next steps.
Sober living and aftercare support
We assist patients and families in identifying appropriate step-down care options, including outpatient therapy, IOP, psychiatry, sober living environments, and 12-step or mutual-aid groups.
